Urinary Incontinence and Prostate Enlargement

Definition

Urinary incontinence is the involuntary leakage of urine, which can significantly affect quality of life. It occurs when even slight pressure on the bladder causes leakage.
It primarily affects middle-aged women who have experienced pregnancy and childbirth or elderly individuals after menopause, but it is increasingly common in younger people as well.

Types

Urinary incontinence is mainly classified into stress incontinence and urge incontinence.
Stress incontinence occurs when urine leaks during activities like coughing or sneezing, while urge incontinence is characterized by a sudden, uncontrollable need to urinate.
Stress incontinence affects 80% of middle-aged patients and is common among women who have been pregnant.
Urge incontinence can occur regardless of age and is increasingly seen in younger individuals.

Prostate Enlargement

Causes

As men approach middle age, symptoms of prostate enlargement may gradually appear, such as a weakened or thin urine stream.
This condition occurs when the prostate tissue enlarges, compressing the urethra or stimulating the bladder above the prostate, or when abnormal tissue growth in the prostate obstructs the urine passage.
Prostate enlargement is a very common condition, affecting 50% of men in their 50s, 60% in their 60s, and 70% in their 70s.

Symptoms of Prostate Enlargement

Enlargement of the prostate compresses nearby structures like the bladder and urethra, causing significant urinary discomfort.
Symptoms can be broadly categorized into those affecting the urethra and those related to bladder irritation.

Urethral Compression Symptoms

  • Delayed Urination: Difficulty starting urination or taking longer to begin
  • Weak Urine Stream (Stranguria): The urine stream becomes weak and thin
  • Post-Micturition Sensation: Feeling of incomplete bladder emptying
  • Split Urination: Urine stream may split into multiple streams or drip intermittently
  • Straining During Urination: Increased effort needed to urinate
  • Prolonged Urination Time: The duration of urination becomes longer
  • Urinary Obstruction: Difficulty urinating or need for catheterization

Bladder Irritation Symptoms

  • Urgency: Sudden, strong need to urinate
  • Frequent Urination: Increased frequency of urination
  • Nocturia: Waking up during the night to urinate
  • Urinary Incontinence: Leakage of urine before reaching the restroom
